HSA/FSA cards. These are Health Savings Accounts that your employer may give you to use towards medial expenses as you see fit. We are set up as a medical account and can run your card the same way as a debit or credit card. We provide detailed receipts with all necessary information for you to keep for your files.

Medical Payments (MedPay) from your auto policy or from the policy of the car you were in during the accident. If you have been in an auto accident and are seeking treatment, we are happy to see you for care and bill your Medpay coverage directly. Medpay is an add-on to your policy that allows you access to treatment after an accident (regardless of who is at-fault). Utilizing this portion of your policy does NOT have an effect on your insurance rates and doesn’t count against you. If you have Medpay, we will bill your carrier directly, which means no up front cost to you.

Workers’ Compensation If you have been injured on the job, we can take on your case and bill your workers’ comp policy directly. This means no cost to you.

TriWest – Community Care (Veterans Affairs) We have recently been accepted as participating providers for TriWest. We are currently waiting for the final approval of all three therapists. Please call our office before scheduling to ensure that we are setup to take your case and have prior authorization from TriWest. A referral from your VA doctor is required. They can fax the referral to us at 520.448.0819 (updated June 2024).

Health Insurance

Arizona has been behind the curve when it comes to health insurance plans covering massage therapy. However, we are seeing subtle changes in the right direction. If you want to know if your plan covers massage therapy, we advise you to call your health insurance carrier and ask them if they cover massage therapy performed by a licensed massage therapist. Be sure to find out about out-of-network benefits as well as in-network benefits. If they do, please contact us so that we can look further into it. When possible, we will make an attempt to bill your health insurance policy when coverage exists and we feel confident that we will get reimbursed.

Aetna – If you are a Costco employee and have Aetna Select in Arizona, your plan covers massage therapy, up to 90 visits per year! Please contact us so that we may assist you in getting authorization to receive care in our office. We are excited to learn that Costco has a plan for it’s employees here in Tucson that cover massage therapy as part of employee healthcare benefits. This is huge and we are really happy that they made the change.

Regardless what health insurance carrier you have, we encourage patients to send in their receipts for massage therapy and ask for reimbursement from their carrier. We will provide you with all of the necessary information (it will be populated on your receipt) to request reimbursement. We have found that some patients do get reimbursed. It really varies by carrier, claims adjuster and your policy. At a minimum, the care should go towards your deductible if you have one.

Out of State Health Insurance If you are from another state and currently in Arizona, check your health insurance policy and ask if you have “Manual Therapy Benefits”. Many other states DO have massage benefits. You can ask if they cover codes such as 97140 and 97124, if provided by a Licensed Massage Therapist — in and out of network. If you do have benefits, ask them what the requirements are, how many sessions are allowed and where the bills should be submitted. We are happy to work with you on this. Please call or email us with any questions at 520.858.0350 or billing@millermedicalmassage.com.

To find out how to submit a Reimbursement Claim to your carrier, login to your online account with your carrier and use the Search tool and enter “Patient Claim Reimbursement Form”. If you don’t have access to an online account then simply use a search engine like Google and enter “Health Insurance Company Name  Patient Claim Reimbursement Form”. For example, if you have United Healthcare, you would search “United Healthcare Patient Claim Reimbursement Form”.

Once you locate the form, download it, print it and fill it out, attach your receipt(s) and a copy of the doctors referral and then mail, fax or email it to their medical claims department. Most of the information they will ask you about will be located on the receipt(s) that we give you. Their contact info should be listed on the reimbursement form including the mailing address, fax number and contact phone number. Then follow up with your carrier via phone every 14 days for an update. The squeaky wheel gets the grease!

Medicare and Supplemental Plans We just learned that certain Medicare Supplemental Plans of Arizona are covering massage therapy as of January 1, 2024. One of them is Blue Cross Blue Shield of Arizona. As of yet, we are not contracted with any Medicare or Medicare Supplement plans. Since some plans cover massage therapy, you will likely be able to get reimbursed if the patient submits the claim. We cannot submit this claim for you. However, we do our best to provide you with the needed information to get reimbursed. Check back frequently to see if we have updated this section and begin taking any Medicare Supplemental plans. (updated June 2024)

Medicaid – Medicaid doesn’t cover massage therapy or manual therapy.

Payment Types We accept cash (preferred), FSA/HSA cards, debit, credit, Venmo and some checks* (*patient must be established and verified).

Packages – We offer several different pre-paid package options that can reduce the cost per session. These are optimal for those who are following a treatment plan in our office.
